Thank you for submitting your forms! You’ve just taken the next step on your fertility journey with Texas Fertility Center, and we’re so excited.

Now that we have your forms, here are a few things to keep in mind:

• If you have a partner, be sure to remind them to complete their forms before your visit. This will give your doctor the background information they need.
• If you can’t make it to your appointment, no worries. Just cancel or reschedule at least two business days before your visit to free up the spot for someone else.
